Built my first vanquish rear axle: 2 Questions

Just built my first vanquish rear axle and I got 2 questions. First not sure if the screws for the upper link mounts are suppose to be included or not but i did not get them. So what size screws are needed to mount the upper link plates to the axle? and 2nd I have some play between the tube and hex nut. did i do something wrong? is this normal? and/or how can this problem be addressed? Thanks for helping another newbie with my first r/c car! Image 1 is right side, Image 2 is left side (looking at from the rear as if it were on the crawler), image 3 is extra bag of hardware either they give extra or mistakenly gave that bag rather then the bag of hardware for upper link mounts?, Image 4 is what i need screws for..

The pin hole on VP axles isn't located properly so the pin doesn't fully seat in the stock hex, you can drill out the hex or buy VP clamping hexes.
 
ordered up. should the hexes on there via pic slide off after removing lock screw or do i disassemble the whole axle to to get hexes off? thanks
